If you are using insurance, the following is required at time of scheduling: upload a valid government issued ID and insurance card, the name of your insurance company or EAP, insurance ID, Group ID, primary insured name, primary insured date of birth, patient name, patient date of birth, patient address. If using an EAP, we need the above plus EAP letter of guarantee of payment. This must be provided at time of scheduling. Thanks.
